Questions:

1. What is the cause of thrombocytopenia in this patient?

Ans- Peripheral destruction of platelets due to immune mechanisms is the most common cause of HIV-associated thrombocytopenia

https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3332279/

2. Pathophysiology of ITP

Ans-

https://ashpublications.org/hematology/article/2008/1/212/95865/Viral-Associated-Immune-Thrombocytopenic-Purpura

Multiple mechanisms may contribute to the development of CITP in the HIV-infected patient and these have recently been reviewed.10 Proposed mechanisms include accelerated platelet clearance due to immune complex disease, and antiplatelet glycoprotein antibodiesand/or anti-HIV antibodies that crossreact with platelet membrane glycoproteins (antigenic mimicry). The ability of the HIV-1 to rapidly mutate may facilitate both its ability to escape immune surveillance and to mimic host antigens. Direct infection of megakaryocytes results in defective platelet production and megakaryocytic apotosis

Psychiatry posting: 27.8.22 to 11.9.22
